CR-CHOP + PD1 inhibitor is a combination therapy regimen used in the treatment of certain lymphomas, particularly non-Hodgkin lymphoma and diffuse large B-cell lymphoma (DLBCL). The "CR" in this context typically refers to "Complete Response," indicating that the regimen is administered with the goal of achieving complete remission. The backbone of this regimen is CHOP chemotherapy, which consists of cyclophosphamide (an alkylating agent), doxorubicin (a DNA intercalator), vincristine (a microtubule inhibitor), and prednisone or prednisolone (a corticosteroid)[3][5][8]. This combination targets rapidly dividing cancer cells through multiple mechanisms. The "+ PD1 inhibitor" component refers to the addition of an immune checkpoint inhibitor targeting programmed cell death protein 1 (PD-1). These inhibitors are monoclonal antibodies that block the interaction between PD-1 on T cells and its ligands on tumor cells, thereby enhancing anti-tumor immune responses. Commonly used PD-1 inhibitors include pembrolizumab and nivolumab. The rationale for combining CHOP with a PD-1 inhibitor is to leverage both cytotoxic chemotherapy and immunotherapy for potentially improved outcomes in aggressive lymphomas. This combination remains investigational for most indications; clinical trials are ongoing to determine efficacy and safety compared to standard regimens such as R-CHOP or G-CHOP[7].
