CRB-15 is a recombinant fusion protein designed as an interleukin-15 (IL-15) receptor antagonist. It consists of a mutant IL-15 molecule linked to an Fc fragment. By competitively binding to the IL-15 receptor without triggering signal transduction, CRB-15 inhibits the proinflammatory effects of IL-15, which is known to drive T-cell proliferation and activation in various autoimmune and inflammatory conditions. Originally developed by Beth Israel Deaconess Medical Center and subsequently acquired by Roche via Cardion, the drug has undergone preclinical evaluation for the treatment of rheumatoid arthritis, inflammatory bowel disease (including colitis), and the prevention of transplant rejection. Preclinical data indicate that CRB-15 can reduce synovitis, bone erosion, and proinflammatory cytokine levels.
