CRD-38 is a novel, subcutaneously administered drug formulation of cannabidiol developed for the treatment of heart failure, particularly heart failure with preserved ejection fraction (HFpEF). The formulation is designed to improve the pharmacokinetic profile of cannabidiol compared to existing oral formulations, potentially allowing for lower doses and less frequent administration while maintaining or enhancing therapeutic efficacy. Preclinical studies have demonstrated that CRD-38 reduces cardiac fibrosis and hypertrophy, improves ejection fraction and cardiac output, and attenuates inflammation and remodeling in models of heart failure. Mechanistically, CRD-38 acts as an agonist at cannabinoid receptors CB1 and CB2 via its cannabidiol component (providing anti-inflammatory effects), while organic nitrates in the compound directly relax vascular smooth muscle—leading to coronary vessel dilation and improved myocardial oxygen supply. Additionally, preservation of mitochondrial function through PPAR-γ activation has been identified as a key mode of action. The drug is currently in preclinical development with IND-enabling activities underway[1][2][3][4][5][6][7].
