Overview

Crovalimab + vitamin K antagonist (VKA) is a combination therapeutic regimen being investigated for the treatment of Antiphospholipid Syndrome (APS). Crovalimab is a humanized, recycling monoclonal antibody developed by Chugai Pharmaceutical (a Roche subsidiary) that targets complement protein C5. It utilizes proprietary antibody engineering to enable pH-dependent binding and enhanced FcRn-mediated recycling, allowing for sustained C5 inhibition with low-volume subcutaneous dosing. Vitamin K antagonists, such as warfarin, are a class of oral anticoagulants that inhibit the vitamin K epoxide reductase complex, thereby reducing the synthesis of active vitamin K-dependent clotting factors (II, VII, IX, and X). This combination is currently being evaluated in Phase II clinical trials (e.g., NCT05963282) as an add-on therapy to standard-of-care anticoagulation to address the role of complement activation in the pathogenesis of thrombotic APS.
