Overview

CRS-3123 is a fully synthetic small molecule antibacterial drug candidate developed for the treatment of *Clostridioides difficile* infection (CDI). It acts as a protein synthesis inhibitor by selectively and potently inhibiting bacterial methionyl-tRNA synthetase (MetRS), an essential enzyme in protein translation. This novel mechanism targets type 1 MetRS found in *C. difficile* and many aerobic Gram-positive bacteria but spares most Gram-negative bacteria and major constituents of the normal intestinal microbiota. CRS-3123 demonstrates high potency against all clinical isolates of *C. difficile*, including epidemic strains such as BI/NAP1/027. Its narrow spectrum may reduce disruption to normal gut flora compared to current therapies. Additional benefits include inhibition of toxin production and sporulation by *C. difficile*, potentially lowering morbidity, mortality, transmission rates, and recurrence risk. Clinical development has reached Phase II with ongoing or planned Phase III studies for CDI[1][2][4][5][6].
