Overview

Crude coal tar is a nearly black, viscous liquid by-product obtained from the destructive distillation of bituminous (black) coal. It is a complex mixture containing phenols, polycyclic aromatic hydrocarbons (PAHs), and heterocyclic compounds. Crude coal tar has been used for decades as a topical treatment for various skin disorders including psoriasis, eczema, seborrheic dermatitis, and dandruff. Its mechanism of action involves keratoplastic effects—normalizing keratin growth in the skin to reduce scaling—as well as antipruritic (anti-itch), anti-inflammatory, and mild antimicrobial properties. Crude forms are less commonly used today due to their strong odor and staining potential; more refined versions are found in creams, ointments, gels, shampoos or bath additives[5][6][10]. In dermatology practice—especially for chronic plaque psoriasis—crude coal tar may be prescribed at concentrations of 2–5% in an emollient base or as part of Goeckerman therapy (coal tar plus UVB phototherapy)[6][9][10]. While effective for symptom relief such as itching and scaling[1][3], crude coal tar can cause side effects like local irritation or photosensitivity; it also carries a theoretical risk of carcinogenicity with high exposure but is generally considered safe at therapeutic doses[6][10].
