Cryopreserved autologous stromal vascular fraction (SVF) is an advanced therapy medicinal product (ATMP) consisting of a heterogeneous cell population derived from a patient's own adipose tissue. The SVF is obtained through the enzymatic digestion of lipoaspirate and contains a mixture of adipose-derived stem cells (ADSCs), endothelial progenitor cells, pericytes, fibroblasts, and various immune cells such as macrophages and lymphocytes. Unlike expanded mesenchymal stem cell products, SVF is not cultured, which preserves the natural cell diversity and microenvironment. The cryopreservation process is a distinctive feature of this formulation, allowing for the storage of the cellular product for subsequent or multiple clinical applications. Developed by the Centre Hospitalier Régional de Marseille (AP-HM), this therapy is primarily investigated for the treatment of refractory ano-perineal fistulas in patients with Crohn's disease. Upon local injection into the fistula tract, the cells exert therapeutic effects through paracrine signaling, secreting a variety of angiogenic and anti-inflammatory factors that promote tissue regeneration, reduce local inflammation, and facilitate the closure of the fistula.
