Overview

Cryopreserved mononuclear cells after extracorporeal photoirradiation refers to a cell-based product derived from the extracorporeal photopheresis (ECP) procedure. ECP involves collecting a patient's white blood cells (mononuclear cells), treating them ex vivo with a photosensitizer, 8-methoxypsoralen (8-MOP), and then exposing them to ultraviolet A (UVA) radiation. This process induces apoptosis in the treated leukocytes, particularly activated T cells. Upon reinfusion into the patient, these apoptotic cells are taken up by antigen-presenting cells (APCs), which is believed to lead to immune modulation, promotion of immune tolerance, and the generation of antigen-specific regulatory lymphocytes (Tregs). The cryopreservation aspect involves freezing these treated cells for later use, a strategy being investigated to improve the accessibility and logistical management of ECP therapy. ECP is an approved treatment for conditions such as cutaneous T-cell lymphoma (CTCL), graft-versus-host disease (GVHD), and cardiac allograft rejection.
