**CT2074** is a sigma-2 receptor modulator developed by Cognition Therapeutics for the treatment of ocular neurodegenerative diseases. It functions as a sigma-2 receptor antagonist with high selectivity (>100-fold over other targets) and nanomolar binding affinity (Ki = 21 nM). The compound has demonstrated significant neuroprotective effects in preclinical glaucoma models, showing substantial preservation of retinal ganglion cells and their axons when administered orally. CT2074 achieves retinal concentrations exceeding 95% receptor occupancy and is being developed primarily for dry age-related macular degeneration, representing a novel therapeutic approach for ocular neuroprotection through sigma-2 receptor modulation.
