Overview

CT2168 is a small molecule sigma-2 receptor (S2R) modulator with high affinity (Ki = 1.4 nM) and >100-fold selectivity over sigma-1 receptor across 116 targets. Developed by Cognition Therapeutics, it reverses trafficking deficits, autophagy impairment, and gene expression changes in preclinical models of Alzheimer's disease (AD) using hAPPsl transgenic mice, restoring pathways like vesicle fusion, synaptic receptor trafficking, and RAB3 regulation toward healthy levels. It also blocks alpha-synuclein oligomer-induced neuronal dysfunction, showing potential disease-modifying effects in synucleinopathies such as Parkinson's disease (PD) and dementia with Lewy bodies (DLB), with preclinical data supporting advancement to IND-enabling studies if superior to lead compound CT1812.
