CTLA4Ig-modified dendritic cells are **dendritic cells engineered to express a fusion protein of cytotoxic T-lymphocyte antigen 4 (CTLA-4) and immunoglobulin (Ig)**, typically via gene transfection or viral vector. The mechanism of action relies on the expression of the CTLA4-Ig fusion protein by the dendritic cells, which binds to **B7 molecules (CD80/CD86) on antigen-presenting cells**, blocking the B7/CD28 costimulatory interaction necessary for T cell activation. This leads to **inhibition of T cell proliferation, induction of T cell apoptosis, and upregulation of regulatory T cells (CD4+CD25+Foxp3+)**, overall resulting in immune tolerance or immunomodulation. Preclinical data support their use as a **cellular immunotherapy** to **promote transplant tolerance** (in islet transplantation models) and as an immunomodulator in autoimmune diseases such as rheumatoid arthritis and potentially in other immune-mediated conditions. These cells play a role in shifting Th1/Th2 balance and enhancing immune regulation in vivo[2][4][6].
