Cultured autologous oral mucosa epithelial sheet (CAOMES) is an advanced therapy medicinal product (ATMP) developed for the treatment of bilateral limbal stem cell deficiency (LSCD). In patients with bilateral disease, autologous limbal stem cells are unavailable for transplantation. This therapy involves harvesting a small biopsy of the patient's own oral mucosa, culturing the epithelial cells ex vivo—often on a scaffold such as denuded amniotic membrane—to form a cohesive sheet, and then transplanting this sheet onto the damaged ocular surface. The transplanted cells act as a surrogate for the missing limbal stem cells, regenerating the corneal epithelium, reducing corneal neovascularization, and improving visual acuity. This approach, often referred to as Cultured Oral Mucosal Epithelial Transplantation (COMET), avoids the need for systemic immunosuppression required for allogeneic grafts.
