Overview

Cultured islet cells refer to isolated clusters of pancreatic endocrine cells (islets of Langerhans) that are maintained in culture prior to transplantation or research use. These preparations are primarily used for the treatment of type 1 diabetes through allogeneic or autologous transplantation. The mechanism involves restoring endogenous insulin production by replacing lost or dysfunctional beta-cells with viable donor-derived or stem cell-derived islets capable of glucose-responsive insulin secretion. Islet culture allows for quality control, viability assessment, and sometimes functional testing before clinical use. Culturing can also reduce immunogenicity and allow time for recipient preparation but may result in some loss of viable cell mass due to apoptosis and stress responses during the process[1][5][7]. Developers include academic consortia (such as the NIH-sponsored Clinical Islet Transplantation Consortium), specialized manufacturing centers, and companies providing research-grade human islets[4][7]. The primary indication remains type 1 diabetes with severe hypoglycemia unawareness or brittle glycemic control.
