Cy5-uPAR-peptide is a fluorescently labeled bicyclic peptide tracer developed by the Netherlands Cancer Institute (NKI-AVL) for the intraoperative visualization of tumors and metastases. The agent specifically targets the urokinase-type plasminogen activator receptor (uPAR), a protein that is significantly overexpressed in various aggressive malignancies, including penile carcinoma and tongue carcinoma, particularly at the invasive tumor front and in metastatic lymph nodes. The peptide's bicyclic structure, stabilized by two disulfide bridges, provides high affinity and specificity for uPAR. When conjugated to the near-infrared fluorophore Cy5, the tracer enables real-time fluorescence-guided surgery, allowing surgeons to identify tumor margins and detect occult micrometastases that might be missed by conventional imaging or visual inspection. Clinical studies at NKI-AVL have focused on its utility in sentinel lymph node biopsies and margin assessment in head and neck and urogenital cancers.
