Cyclodextrin polysulphate (CDPS) is a semi-synthetic sulfated oligosaccharide derivative of cyclodextrin being investigated for its potential as a disease-modifying osteoarthritis drug (DMOAD). It exhibits chondroprotective and anti-inflammatory properties by inhibiting cartilage degradation and osteophyte formation. Preclinical studies in animal models, such as the rabbit anterior cruciate ligament transection (ACLT) model, have demonstrated that systemic administration of CDPS can significantly reduce cartilage degradation and inhibit osteophyte formation. Its mechanism of action is believed to involve the inhibition of matrix-degrading enzymes, including matrix metalloproteinases (MMPs) and aggrecanases (ADAMTS), thereby protecting the structural integrity of joint cartilage.
