Cyclophosphamide + ATG (rabbit anti-thymocyte globulin) is a non-myeloablative conditioning regimen utilized in autologous hematopoietic stem cell transplantation (aHSCT) for the treatment of active relapsing-remitting multiple sclerosis (RRMS). The regimen combines cyclophosphamide, a nitrogen mustard alkylating agent that interferes with DNA replication, and ATG, a polyclonal antibody biologic derived from rabbits immunized with human thymocytes that causes profound T-cell depletion. This protocol is designed to "reset" the immune system by eradicating the pathogenic lymphocytes responsible for MS-related neuroinflammation, followed by the reinfusion of the patient's own stem cells to restore immune function. It is currently being studied by Scripps Health in clinical trials (e.g., NCT05482542) to evaluate its efficacy and safety profile compared to other conditioning protocols like cyclophosphamide plus rituximab.
