Overview

This drug is a combination therapy consisting of four agents used primarily in the treatment of breast cancer, including triple-negative and hormone receptor-positive/HER2-negative subtypes. The regimen combines chemotherapy agents cyclophosphamide, doxorubicin, and paclitaxel with the immune checkpoint inhibitor durvalumab. - **Cyclophosphamide** is an alkylating agent that interferes with DNA replication by cross-linking DNA strands, leading to cell death. - **Doxorubicin** (an anthracycline) intercalates into DNA and inhibits topoisomerase II, causing DNA damage and apoptosis. - **Paclitaxel** (a taxane) stabilizes microtubules and prevents their disassembly, inhibiting mitosis. - **Durvalumab** is a monoclonal antibody targeting PD-L1 (programmed death-ligand 1), blocking its interaction with PD-1 receptors on T cells to enhance anti-tumor immune response. This combination has been studied as neoadjuvant therapy for stage I-III triple-negative breast cancer as well as high-risk hormone receptor-positive/HER2-negative breast cancer. Clinical trials have shown that adding durvalumab to the chemotherapy backbone may improve pathologic complete response rates compared to chemotherapy alone. The regimen involves sequential or concurrent administration of these drugs over several cycles before surgery[1][3][4][8].
