Chemotherapy + Zevalin is a sequential combination regimen primarily investigated for the treatment of diffuse large B-cell lymphoma (DLBCL). The regimen consists of induction therapy with standard CHOP chemotherapy—a combination of cyclophosphamide (an alkylating agent), doxorubicin (an anthracycline), vincristine (a microtubule inhibitor), and prednisone (a corticosteroid)—followed by consolidation with Yt90-Zevalin (yttrium-90 ibritumomab tiuxetan). Ibritumomab tiuxetan is a murine IgG1 kappa monoclonal antibody that targets the CD20 antigen expressed on the surface of B-lymphocytes. It is chelated to the radioisotope yttrium-90, a pure beta-emitter. This radioimmunotherapy approach delivers targeted radiation directly to tumor cells, utilizing a 'crossfire effect' where the beta particles destroy both the antibody-bound cells and adjacent malignant cells within a short radius, thereby addressing residual disease after initial chemotherapy.
