This combination regimen consists of cyclophosphamide, bortezomib, and maraviroc, and is being evaluated by the National Cancer Institute (NCI) for the prophylaxis of graft-versus-host disease (GVHD) in people living with HIV (PLWH) who are undergoing HLA-haploidentical hematopoietic cell transplantation for hematologic malignancies. Cyclophosphamide is used as a post-transplant immunosuppressant to selectively deplete alloreactive T cells. Bortezomib, a proteasome inhibitor, modulates immune responses and induces apoptosis in activated T cells. Maraviroc, a CCR5 antagonist, serves a dual role by maintaining HIV viral suppression and blocking CCR5-mediated T-cell trafficking to GVHD target organs such as the gut and liver.
