This is a combination chemotherapy and supportive care regimen consisting of four cytotoxic agents—cyclophosphamide, docetaxel, doxorubicin, and two granulocyte colony-stimulating factors (G-CSFs), filgrastim and lenograstim. Cyclophosphamide is an alkylating agent that crosslinks DNA to inhibit cell division. Docetaxel is a taxane that stabilizes microtubules, preventing their depolymerization and thereby inhibiting mitosis. Doxorubicin is an anthracycline antibiotic that intercalates into DNA, inhibits topoisomerase II, and generates free radicals leading to cytotoxicity. Filgrastim and lenograstim are recombinant G-CSFs used to stimulate neutrophil production in the bone marrow to reduce the risk of chemotherapy-induced neutropenia[1][5][6][8][10]. This combination may be used in high-risk or dose-intensive regimens for breast cancer or other malignancies where severe myelosuppression is anticipated.
