Overview

This is a combination chemotherapy regimen consisting of three cytotoxic agents—cyclophosphamide, doxorubicin (also known as Adriamycin), and methotrexate. Each drug has a distinct mechanism of action targeting rapidly dividing cancer cells. Cyclophosphamide is an alkylating agent that crosslinks DNA, leading to cell death. Doxorubicin is an anthracycline antibiotic that intercalates into DNA and inhibits topoisomerase II, resulting in inhibition of DNA replication and transcription. Methotrexate is an antimetabolite that inhibits dihydrofolate reductase, blocking the synthesis of tetrahydrofolate required for purine and thymidylate synthesis, thereby inhibiting DNA synthesis. This combination has been used primarily in the treatment of various cancers including advanced non-small cell lung cancer (NSCLC) and lymphomas[1][2][6]. The regimen may be referred to as part of multi-agent protocols such as MACC (methotrexate, doxorubicin/Adriamycin, cyclophosphamide, CCNU/lomustine) or CODOX-M when combined with other agents for specific indications[1][2][6].
