Overview

Cyclophosphamide + doxorubicin + prednisone + etoposide (commonly abbreviated as the CHEP regimen) is a four-drug combination chemotherapy regimen used primarily in the treatment of aggressive B-cell and T-cell lymphomas, such as peripheral T-cell lymphoma (PTCL) and adult T-cell leukemia/lymphoma (ATLL). The regimen is built upon the standard CHOP (cyclophosphamide, doxorubicin, vincristine, prednisone) backbone, substituting etoposide for vincristine or adding etoposide to CHP (cyclophosphamide, doxorubicin, prednisone) to enhance efficacy, particularly in T-cell malignancies. Cyclophosphamide acts as an alkylating agent; doxorubicin is an anthracycline topoisomerase II inhibitor and DNA intercalator; etoposide is a podophyllotoxin derivative that inhibits topoisomerase II; and prednisone is a synthetic corticosteroid that provides anti-inflammatory and immunosuppressive effects. CHEP is frequently evaluated in clinical trials in combination with targeted agents, such as the anti-CD30 antibody-drug conjugate brentuximab vedotin (BV-CHEP).
