CHOP + granulocyte-colony stimulating factor (G-CSF) is a dose-dense chemotherapy regimen used as a standard of care for various aggressive non-Hodgkin lymphomas, including peripheral T-cell lymphoma (PTCL). The regimen consists of the CHOP backbone—cyclophosphamide (an alkylating agent), doxorubicin (an anthracycline topoisomerase II inhibitor), vincristine (a microtubule inhibitor), and prednisone (a corticosteroid)—administered on a 14-day cycle (CHOP-14). To mitigate the severe neutropenia associated with this shortened dosing interval, a G-CSF such as pegfilgrastim is administered to stimulate the production and maturation of neutrophils by activating the G-CSF receptor on hematopoietic progenitor cells. This combination was utilized as the control arm in the ACT-1 Phase III trial (NCT00646854), which evaluated the addition of alemtuzumab to the regimen in young patients with previously untreated systemic PTCL.
