Overview

This is a combination chemotherapy regimen consisting of four cytotoxic agents—cyclophosphamide, etoposide, doxorubicin, and carboplatin. Each drug has a distinct mechanism of action targeting cancer cells at different points in the cell cycle or through different cellular processes: - Cyclophosphamide is an alkylating agent that crosslinks DNA strands, leading to inhibition of DNA replication and cell death. - Etoposide is a topoisomerase II inhibitor that prevents the re-ligation of DNA strands during replication, causing double-strand breaks and apoptosis. - Doxorubicin is an anthracycline antibiotic that intercalates into DNA and inhibits topoisomerase II while also generating free radicals, resulting in cytotoxicity. - Carboplatin is a platinum-based compound that forms intra- and inter-strand DNA crosslinks, disrupting DNA synthesis. This combination has been investigated primarily for use in pediatric oncology (such as Wilms tumor) as well as for certain adult cancers including small-cell lung cancer. The rationale for combining these agents lies in their non-overlapping mechanisms of action and toxicity profiles[3][1]. The regimen aims to maximize antitumor efficacy while managing cumulative toxicities.
