Overview

This is a combination chemotherapy regimen consisting of four agents: - Cyclophosphamide, an alkylating agent that crosslinks DNA, leading to cell death. - Etoposide, a topoisomerase II inhibitor that induces DNA strand breaks and apoptosis. - Filgrastim (granulocyte colony-stimulating factor), a recombinant protein that stimulates the production of neutrophils to reduce the risk and duration of chemotherapy-induced neutropenia. - Mitoxantrone, an anthracenedione antineoplastic agent that intercalates into DNA and inhibits both DNA and RNA synthesis as well as topoisomerase II. This combination is used in high-dose or salvage regimens for hematologic malignancies such as lymphoma or acute myeloid leukemia (AML), often in settings where intensive myelosuppression is anticipated. Filgrastim is included specifically to mitigate severe neutropenia caused by the cytotoxic agents. The regimen has been studied in advanced breast carcinoma[1], refractory lymphoma[8], AML[5], and other aggressive cancers. Toxicities are primarily hematologic (notably neutropenia and thrombocytopenia), with nonhematologic effects including nausea, vomiting, alopecia, fatigue, and potential cardiac toxicity from mitoxantrone.
