This is a combination conditioning regimen used primarily in allogeneic hematopoietic stem cell transplantation (allo-HSCT), especially for severe aplastic anemia. It consists of three agents: - Cyclophosphamide, an alkylating agent that crosslinks DNA and suppresses the immune system. - Fludarabine, a purine analog that inhibits DNA synthesis and has immunosuppressive properties. - Anti-thymocyte globulin (ATG), a polyclonal antibody preparation targeting human T lymphocytes to induce profound immunosuppression by depleting T cells. The combination is designed to provide effective immunosuppression to prevent graft rejection and facilitate engraftment while reducing the risk of graft-versus-host disease (GVHD). This regimen is often used as an alternative to total body irradiation-based protocols. The main indications are conditioning prior to allogeneic stem cell transplantation for severe aplastic anemia and other hematologic disorders[1][2][3][4].
