Cyclophosphamide + GVAX Pancreas Vaccine + CRS-207 is an investigational heterologous prime-boost immunotherapy regimen designed for the treatment of metastatic pancreatic adenocarcinoma. The strategy utilizes low-dose cyclophosphamide to deplete immunosuppressive regulatory T cells (Tregs), followed by the GVAX Pancreas vaccine to prime the immune system. GVAX is an allogeneic whole-cell vaccine consisting of pancreatic tumor cell lines engineered to secrete granulocyte-macrophage colony-stimulating factor (GM-CSF), which promotes dendritic cell recruitment and antigen presentation. The boost phase utilizes CRS-207, a live-attenuated strain of *Listeria monocytogenes* engineered to express mesothelin, a tumor-associated antigen highly prevalent in pancreatic cancer. This combination aims to generate a robust, mesothelin-specific T-cell response. Developed by Aduro Biotech in collaboration with Johns Hopkins University, the regimen reached Phase IIb clinical testing (ECLIPSE study) but failed to demonstrate a survival benefit over standard chemotherapy, leading to the discontinuation of this specific triplet combination.
