Cyclophosphamide + ifosfamide is a combination of two oxazaphosphorine alkylating agents used in chemotherapy regimens for various cancers. Both drugs are prodrugs that require hepatic activation to form cytotoxic metabolites (phosphoramide mustard for cyclophosphamide and ifosfamide mustard for ifosfamide). These active metabolites alkylate DNA, forming inter- and intra-strand crosslinks at the N-7 position of guanine, which inhibits DNA replication and transcription, leading to cell death. Both agents are cell cycle phase non-specific but act primarily by disrupting DNA synthesis. They also have immunosuppressive properties. This combination has been studied as neoadjuvant therapy in locally advanced non-small-cell lung cancer and is used in other malignancies such as sarcomas, lymphomas, testicular cancer, bladder cancer, ovarian cancer, breast cancer, and more[1][2][3][4][6][8].
