Overview

Cyclophosphamide + ixabepilone is a combination chemotherapy regimen used primarily in the neoadjuvant treatment of locally advanced, HER2-negative breast cancer. Cyclophosphamide is an alkylating agent that interferes with DNA replication and transcription, leading to cell death. Ixabepilone is a semisynthetic analog of epothilone B and acts as a microtubule stabilizer by binding directly to beta-tubulin subunits, suppressing their dynamic instability, blocking mitosis, and inducing apoptosis. This combination has been investigated as an alternative to taxane-based regimens due to ixabepilone’s activity in taxane-resistant tumors. Clinical studies have shown this regimen yields pathologic complete response rates similar to other neoadjuvant chemotherapies[1][2]. The most common significant toxicity observed is neutropenia.
