Overview
Cyclophosphamide + methotrexate is a combination immunosuppressive regimen being evaluated in the CY-MET-RIC clinical trial for the prophylaxis of graft-versus-host disease (GVHD). Methotrexate is a small molecule antimetabolite that inhibits dihydrofolate reductase (DHFR), thereby interfering with DNA synthesis and cell proliferation. Cyclophosphamide is an alkylating agent that cross-links DNA, and when administered in the post-transplant setting (PTCY), it selectively eliminates proliferating alloreactive T cells while sparing regulatory T cells and non-alloreactive memory T cells. This combination is specifically being tested in patients with hematologic malignancies undergoing reduced-intensity conditioning (RIC) allogeneic hematopoietic stem cell transplantation to improve outcomes and reduce the incidence of acute and chronic GVHD compared to standard prophylaxis strategies.
