cyclophosphamide + pemetrexed + 5-fluorouracil is a combination chemotherapy regimen being evaluated by West China Hospital in a Phase 2 clinical trial. This regimen is part of a precision medicine study where treatment selection for locally advanced or metastatic poorly differentiated or anaplastic thyroid cancer is guided by in-vitro drug sensitivity testing using tumor-derived organoids. The combination consists of cyclophosphamide, an alkylating agent that cross-links DNA; pemetrexed, a multi-targeted antifolate that inhibits enzymes involved in purine and pyrimidine synthesis; and 5-fluorouracil, a pyrimidine analog that inhibits thymidylate synthase. This approach aims to improve outcomes in rare and aggressive thyroid cancers that have failed conventional therapies.
