Overview

This is a combination chemotherapy regimen consisting of four drugs: - Cyclophosphamide (an alkylating agent) - Pirarubicin (an anthracycline antibiotic and analogue of doxorubicin) - Prednisolone (a synthetic glucocorticoid corticosteroid) - Vincristine (a vinca alkaloid) The combination is primarily used in the treatment of non-Hodgkin's lymphoma. Cyclophosphamide acts by cross-linking DNA strands and inhibiting cell division. Pirarubicin intercalates into DNA and inhibits topoisomerase II, leading to DNA damage and apoptosis; it is considered less cardiotoxic than doxorubicin. Vincristine disrupts microtubule formation during mitosis, arresting cell division. Prednisolone exerts anti-inflammatory and immunosuppressive effects as well as direct cytotoxicity against certain lymphoid malignancies. This regimen has demonstrated efficacy as first-line therapy for non-Hodgkin's lymphoma with an acceptable toxicity profile—mainly leukopenia—with mild alopecia and low observed cardiotoxicity[1][4]. The combination may be referred to as "VEP-THP" or similar acronyms in clinical literature.
