Overview

PT + CY + FK is a combination graft-versus-host disease (GVHD) prophylaxis regimen used in allogeneic hematopoietic stem cell transplantation (allo-HSCT). The regimen consists of post-transplantation cyclophosphamide (PTCy) and tacrolimus (FK506). Cyclophosphamide is a cytotoxic alkylating agent that, when administered post-transplant, selectively depletes alloreactive T cells while sparing regulatory T cells and hematopoietic stem cells. Tacrolimus is a calcineurin inhibitor that suppresses T-cell activation by inhibiting the phosphatase activity of calcineurin. This specific combination has been evaluated in clinical trials conducted by the Shanghai Jiao Tong University School of Medicine, particularly for adult patients with lymphoid malignancies such as acute lymphoblastic leukemia and T-cell lymphoma undergoing peripheral stem cell transplantation. The regimen is sometimes further modified with the addition of low-dose anti-thymocyte globulin (ATG) for unrelated or haplo-identical donor transplants.
