This refers to a graft-versus-host disease (GVHD) prophylaxis regimen combining post-transplantation cyclophosphamide (PT-Cy), tacrolimus (FK506), and low-dose anti-thymocyte globulin (ATG). Developed and evaluated by the Shanghai Jiao Tong University School of Medicine, specifically at Rui Jin Hospital, this combination is used for patients with lymphoid malignancies undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT). The regimen aims to selectively deplete alloreactive T-cells while preserving regulatory T-cells and graft-versus-leukemia effects. Cyclophosphamide acts as an alkylating agent that targets proliferating T-cells, tacrolimus serves as a calcineurin inhibitor to suppress T-cell activation, and ATG is a polyclonal antibody that provides further T-cell depletion. Clinical trials, such as NCT04118075, have demonstrated that this triple-drug combination can effectively reduce the incidence of acute GVHD in high-risk patients with conditions like acute lymphoblastic leukemia and T-cell lymphoma.
