Overview

Cyclophosphamide + topotecan + bevacizumab is a combination chemotherapy regimen used primarily in the treatment of relapsed or refractory cancers, including Ewing's sarcoma and ovarian cancer. Cyclophosphamide is an alkylating agent that interferes with DNA replication, leading to cell death. Topotecan is a topoisomerase I inhibitor that prevents DNA unwinding and replication in cancer cells. Bevacizumab is a monoclonal antibody targeting vascular endothelial growth factor (VEGF), thereby inhibiting angiogenesis—the formation of new blood vessels necessary for tumor growth and metastasis[3][6]. This combination leverages cytotoxic effects on tumor cells (cyclophosphamide, topotecan) alongside anti-angiogenic therapy (bevacizumab) to enhance anti-tumor efficacy[3][5]. The regimen has been studied in pediatric populations for Ewing's sarcoma and neuroblastoma, as well as in adults with advanced ovarian cancer who have failed multiple prior therapies[1][2][3].
