Overview

Cyclophosphamide Sandoz is a branded generic version of cyclophosphamide, a nitrogen mustard alkylating agent and potent immunosuppressant. As a prodrug, it is metabolically activated by hepatic cytochrome P450 enzymes, primarily CYP2B6, into active metabolites such as phosphoramide mustard. This active form creates interstrand and intrastrand DNA cross-links at the N-7 position of guanine, which inhibits DNA replication and triggers apoptosis in rapidly dividing cells, including lymphocytes. While traditionally used as a chemotherapy agent for various malignancies like lymphomas and leukemias, its immunosuppressive properties are leveraged in the treatment of severe autoimmune conditions and paraneoplastic syndromes. Specifically, Assistance Publique Hopitaux De Paris has investigated its use in treating paraneoplastic sensory neuropathy associated with anti-Hu antibodies, where it aims to suppress the immune-mediated destruction of dorsal root ganglion neurons.
