Cyclosporin + diclofenac is a combination of two pharmaceutical agents used in certain clinical scenarios, particularly in the management of severe, treatment-refractory rheumatoid arthritis. Cyclosporin is an immunosuppressant that acts as a calcineurin inhibitor to suppress T cell activation by inhibiting the dephosphorylation and activation of nuclear factor of activated T cells (NF-AT), thereby reducing cytokine production and immune response. Diclofenac is a nonsteroidal anti-inflammatory drug (NSAID) that inhibits cyclooxygenase enzymes (COX-1 and COX-2), leading to decreased synthesis of prostaglandins involved in inflammation and pain signaling. When used together, there is a significant pharmacokinetic interaction—cyclosporin increases systemic exposure to diclofenac, potentially doubling its blood levels. This combination requires careful monitoring due to increased risk for nephrotoxicity; however, with appropriate dose titration and clinical monitoring, it can be safely administered for select patients[2][3][5].
