Overview

This is a combination immunosuppressive regimen consisting of three agents—cyclosporine, mycophenolate mofetil, and tacrolimus—used primarily to prevent organ rejection in transplant recipients. Each drug has a distinct mechanism of action targeting different aspects of the immune response: - **Cyclosporine** is a calcineurin inhibitor that suppresses T-cell activation by inhibiting interleukin-2 production. - **Tacrolimus** is also a calcineurin inhibitor but binds to FK506-binding protein, leading to inhibition of T-cell activation. - **Mycophenolate mofetil** inhibits inosine monophosphate dehydrogenase (IMPDH), blocking de novo guanosine nucleotide synthesis and thus lymphocyte proliferation. The combination aims to provide potent immunosuppression while minimizing toxicity by using lower doses of each agent. This regimen is most commonly used in solid organ transplantation (such as kidney transplants) for both induction and maintenance phases[1][7][8]. The use of multiple agents with complementary mechanisms helps reduce the risk of acute rejection and allows for individualized dosing based on patient response.
