Cynanchum wilfordii extract is a botanical preparation derived from the roots of the *Cynanchum wilfordii* plant, which is traditionally used in East Asian medicine for vascular and inflammatory conditions. Research indicates that the extract, particularly ethanol-based preparations, exerts vasoprotective effects by improving the nitric oxide/cyclic GMP signaling pathway and restoring endothelial nitric oxide synthase (eNOS) expression. It demonstrates anti-atherosclerotic activity by inhibiting the expression of endothelin-1 (ET-1) and key cellular adhesion molecules, including E-selectin, vascular cell adhesion molecule-1 (VCAM-1), and intercellular adhesion molecule-1 (ICAM-1). Additionally, it has been shown to improve lipid profiles by reducing low-density lipoprotein (LDL) and increasing high-density lipoprotein (HDL) cholesterol levels.
