Cytomegalovirus pp65-specific cytotoxic T lymphocytes are a form of adoptive cell therapy consisting of ex vivo expanded or selected human T cells that specifically recognize the phosphoprotein 65 (pp65) antigen from human cytomegalovirus (CMV). These CTLs are generated by stimulating donor peripheral blood mononuclear cells with peptides derived from the immunodominant CMV antigen pp65. The resulting product is enriched for CD8+ and CD4+ effector T cells capable of recognizing and killing CMV-infected target cells via perforin/granzyme-mediated apoptosis. This therapy is primarily used to restore or enhance cellular immunity against CMV in immunocompromised patients, such as those undergoing allogeneic hematopoietic stem cell transplantation, where reactivation or primary infection with CMV can cause significant morbidity and mortality. The mechanism of action involves direct recognition and lysis of infected host cells presenting the pp65 antigen in the context of HLA molecules[1][2][3][5].
