D-cycloserine + behavioral therapy is a combination intervention developed by Duke University for the treatment of tobacco use disorder, specifically targeting light smokers. The pharmacological component, D-cycloserine (DCS), is a small molecule that acts as a partial agonist at the glycine-binding site of the N-methyl-D-aspartate (NMDA) receptor. By modulating NMDA receptor activity, DCS is thought to enhance synaptic plasticity and facilitate the extinction of conditioned responses. In this intervention, DCS is used as an adjunct to behavioral counseling, which includes enhanced cue exposure treatment (CET) and interactive SMS-based support. The goal of this combined approach is to accelerate the extinction of smoking-related cues and improve cessation outcomes. The intervention was evaluated in the Project Q pilot study at Duke University.
