d-Larazotide is a synthetic peptide derivative of larazotide (AT-1001) that incorporates d-amino acids to enhance metabolic stability and resistance to proteolytic degradation in the gastrointestinal tract. Developed by Interlude Biopharma, it functions as a zonulin receptor antagonist and tight junction regulator. The drug is designed to block the zonulin signaling pathway, which otherwise triggers the opening of intestinal tight junctions and increases paracellular permeability (often referred to as "leaky gut"). While the original l-amino acid version, larazotide acetate, reached Phase III clinical trials for celiac disease, d-larazotide is currently in the IND-enabling (pre-IND) stage of development for undisclosed indications, likely building upon the clinical path established by its predecessor.
