D0181 is a chimeric antigen receptor (CAR) T-cell therapy candidate targeting mesothelin (MSLN), a cell-surface glycoprotein overexpressed in various solid malignancies. Developed by Lentigen Technology (a Miltenyi Biotec company), D0181 utilizes a second-generation CAR architecture featuring a single-chain variable fragment (scFv) specific for the membrane-proximal domain of MSLN, coupled with a 4-1BB co-stimulatory domain and a CD3ζ signaling domain. This specific targeting of the membrane-proximal region is intended to maintain efficacy in the presence of shed MSLN, which can otherwise act as a decoy for CAR-T cells. Preclinical studies presented at AACR 2025 demonstrated that D0181 achieves superior in vivo expansion and persistence compared to CD28-based MSLN CAR-T cells, leading to effective tumor control in pancreatic cancer models.
