Overview

D10NS is a hypertonic intravenous solution consisting of 10% dextrose in normal saline (0.9% sodium chloride). It contains 10 grams of dextrose per 100 mL of normal saline, making it more concentrated than standard D5NS. As a hypertonic solution, it must be administered carefully, preferably through a large vein to minimize venous irritation. **Clinical Uses:** * Treatment of hypoglycemia (blood sugar < 60 mg/dL or 60-80 mg/dL with symptoms) * Management of patients in PEA (Pulseless Electrical Activity) or asystole with suspected hypoglycemia * Providing both calories (from dextrose) and electrolytes (from normal saline) simultaneously * As a source of water and calories for patients who cannot take oral fluids **Administration:** * Typical adult dosage for hypoglycemia: 25 grams of dextrose in 250 mL solution administered intravenously. * Pediatric dosage: 5 mL/kg of D10 solution (maximum 250 mL) intravenously or intraosseously. * Administered through a peripheral IV or intraosseous (IO) line. * For peripheral administration, recommended to use a small-bore needle into a large vein to minimize irritation. **Precautions and Monitoring:** * Monitor the IV site for extravasation. * Monitor blood glucose levels and mental status frequently (e.g., every 5 minutes). * Potential side effects including hyperglycemia, warmth/burning from IV injection, diuresis, thrombophlebitis, and tissue necrosis if IV/IO infiltrates. * Consider central venous access for long-term administration of hypertonic dextrose solutions. **Contraindications:** * Patients with known hypersensitivity to dextrose, corn, or corn products. * Patients with hyperglycemia. * Use with caution in patients at risk for fluid overload, electrolyte imbalances, or those with renal impairment.
