D4-IgG4H-CD28TM CAR-T is an experimental chimeric antigen receptor (CAR) T-cell therapy developed by researchers at the National Institutes of Health (NIH) for the treatment of pancreatic cancer. The therapy utilizes a dromedary camel-derived VHH nanobody (clone D4) to target glypican-1 (GPC1), an oncofetal antigen frequently overexpressed in pancreatic ductal adenocarcinoma. The CAR construct is specifically engineered with a structurally rigid IgG4 hinge and a CD28 transmembrane domain, which facilitates CAR dimerization and enhances T-cell signaling, particularly in tumors with low antigen density. Preclinical studies have demonstrated tumor regression in orthotopic pancreatic cancer models and identified specific gene signatures, such as ID1, associated with high T-cell polyfunctionality.
