d5-ethyl-beta-d-glucuronide is a deuterated exogenous volatile organic compound (EVOC) probe developed by Owlstone Medical for the non-invasive detection of lung cancer. The compound consists of a glucuronic acid moiety conjugated to a deuterated ethyl group. It is designed to be administered intravenously, where it serves as a substrate for beta-glucuronidase, an enzyme that is significantly overexpressed in the microenvironment of various solid tumors, including lung cancer. Upon cleavage by the enzyme, the probe releases deuterated ethanol (ethanol-d5), which is volatile and enters the systemic circulation. The ethanol-d5 is subsequently eliminated through the lungs and can be detected in the patient's breath using Owlstone's Breath Biopsy platform, which utilizes Field Asymmetric Ion Mobility Spectrometry (FAIMS) and Gas Chromatography-Mass Spectrometry (GC-MS). This metabolic approach allows for the real-time, non-invasive assessment of tumor-specific enzymatic activity, facilitating early diagnosis and potentially monitoring treatment response in lung cancer patients.
