D8 chimeric antigen receptor T cells are an autologous CAR-T cell therapy featuring a novel, high-affinity Fab-based binder (the D8 clone) that specifically targets the B-cell maturation antigen (BCMA). Developed by researchers at University College London (UCL) in collaboration with Autolus Therapeutics, the D8 binder was selected via functional screening for its superior sensitivity to cells with low surface BCMA density compared to first-generation binders like bb2121. This high-sensitivity construct is designed to maximize the depth of clinical response and prevent relapse due to antigen escape in patients with plasma cell dyscrasias. The therapy is currently being evaluated in Phase 1 clinical trials (notably the MCARTY and ALARIC studies) for the treatment of relapsed or refractory multiple myeloma and AL amyloidosis. Additionally, the D8 CAR construct serves as the BCMA-targeting component of the dual-targeting CAR-T product known as AUTO8.
