DA-5218-C is a fixed-dose combination (FDC) drug developed by Dong-A ST Co., Ltd. for the treatment of type 2 diabetes mellitus. It consists of two active pharmaceutical ingredients: evogliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, and metformin, a biguanide. Evogliptin works by inhibiting the DPP-4 enzyme, which prevents the degradation of incretin hormones such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP), thereby increasing insulin secretion and decreasing glucagon levels in a glucose-dependent manner. Metformin primarily reduces hepatic glucose production and improves peripheral insulin sensitivity, partially through the activation of AMP-activated protein kinase (AMPK) and inhibition of mitochondrial complex I. DA-5218-C is frequently utilized as a dual-therapy comparator in clinical trials evaluating Dong-A ST's triple-combination therapy, DA-5218 (evogliptin/dapagliflozin/metformin), particularly in patients whose glycemic levels are inadequately controlled by metformin monotherapy.
