dabigatran + apixaban

Dabigatran + apixaban is a hypothetical or off-label combination of two direct oral anticoagulants (DOACs): dabigatran and apixaban. Dabigatran is a direct thrombin (factor IIa) inhibitor, while apixaban is a selective factor Xa inhibitor. Both drugs are approved individually for the prevention and treatment of thromboembolic disorders such as nonvalvular atrial fibrillation (NVAF), deep vein thrombosis (DVT), and pulmonary embolism (PE)[2][3][4][7]. They act by inhibiting different steps in the coagulation cascade—dabigatran blocks thrombin to prevent fibrin formation, whereas apixaban inhibits factor Xa to block conversion of prothrombin to thrombin[2][3]. There are no clinical guidelines or approvals for their combined use due to an extremely high risk of bleeding; concurrent administration is generally contraindicated[5].
