Overview

This is a combination regimen of three small molecule kinase inhibitors: dabrafenib, lorlatinib, and trametinib. Dabrafenib is a BRAF inhibitor that targets mutant BRAF proteins, particularly the V600E mutation, blocking aberrant MAPK pathway signaling. Trametinib is a MEK1/2 inhibitor that further suppresses the MAPK pathway downstream of BRAF. Lorlatinib is an ALK and ROS1 tyrosine kinase inhibitor used primarily in non-small cell lung cancer (NSCLC) with ALK or ROS1 rearrangements. This triple combination has been reported as an experimental therapy for patients with advanced NSCLC harboring both ROS1 rearrangement and acquired BRAF V600E mutation after resistance to prior targeted therapies[1]. The rationale for this regimen is to overcome resistance mechanisms by simultaneously inhibiting multiple oncogenic drivers.
